STEP 2: Once a space for your child has been confirmed, you will need to fill out the age-appropriate Admission Agreement, and all 3 of the other forms below:

• Admission Agreement for 3-year-olds: Agreement-Weds-2018-19
  
• Admission Agreement for 4 to 5-year-olds: Tues-Thurs-Agreement-2018-19
  
• Admission Agreement for Friday Program: Friday-Agreement-2018-19
  
• EMERGENCY FORM — This MUST have a photo of the child attached 
• CONSENT FOR MEDICAL EMERGENCY
• LIABILITY WAIVER

STEP 3: All fees are to be paid through Curacubby’s automatic and secure services. You will receive instructions on how to enroll in this payment service.
